The Documents section of the patient chart stores files: lab results, imaging reports, referral letters, insurance cards, and more. This article covers uploading, categorization, and sharing.
Uploading documents
From the patient chart, go to Documents → Upload. Drag and drop files or click to browse. Supported formats: PDF, JPG, PNG, TIFF. Max file size is 50 MB. You can upload multiple files at once. Each file needs a category and optional description.
Categories
Common categories: Lab results, Imaging, Referral letters, Insurance cards, Consent forms, ID documents, Correspondence. Categories are used for filtering and determine retention rules. Admins can add custom categories under Establishment → Settings → Document categories.
Organizing
Documents are sorted by date (newest first). Use the filter to show only specific categories or date ranges. Star important documents to pin them to the top. You can rename documents and edit descriptions after upload.
Sharing with patients
Check "Share with patient" when uploading to make the document visible in their portal. Patients see shared documents under Records. You can also share existing documents retroactively — open the document and toggle the share switch. Sharing is auditable and appears in the access log.
Downloading and printing
Click any document to preview it in the browser. Use the download button to save a copy. For PDFs, you can print directly from the preview. Bulk download: select multiple documents with checkboxes, then click Download selected to get a ZIP file.
Retention and purging
Documents are retained according to your state's medical record laws (typically 7-10 years after last visit). Admins can set auto-purge rules by category under Establishment → Settings → Retention. Purged documents are soft-deleted and recoverable for 90 days, then permanently deleted.
