Logging a 3 or lower shows the 988 lifeline - please reach out, do not just log

Slide a number, optionally add a sentence - your care team sees the trend, not just the latest dot

Open My wellness → Mood. Slide the dial from 1 ("Terrible") to 10 ("Amazing"). Add a short note if something specific is on your mind. Submit. The entry timestamps to the moment you submitted, not midnight, and the trend bar at the bottom is your last seven days at a glance.

Multiple entries per day

You can log as many times a day as you want — useful when something shifts (after a difficult conversation, after a workout). If you already logged today, a heads-up appears so you do not accidentally double-submit; tap through to add another on purpose.

When the score is low

If you log a 3 or lower, you will see a banner with the 988 Suicide and Crisis Lifeline (call, text, or chat) and a reminder to go to the ER for an immediate emergency. Logging a low number is not a substitute for reaching out — please do both.

Who sees your entries

Your care team at this clinic. Mood entries appear in your chart as journal items — your provider can spot trends across days and weeks and bring them up at your next visit. They are not shared with insurance, employer, or any third party.
