The journal is a place to write whatever you would otherwise lose between visits — a side effect that started Tuesday, a food that triggered a flare, the night you slept five hours. It is your space; the format is up to you.

Slept poorly, mild headache through the morning

Woke up at 3am, fell back asleep around 5. Headache started by 9 and faded after lunch. New medication started Monday - wondering if related. No nausea.

Private by default - toggle Share to add an entry to your chart so your provider can read it before your next visit

My wellness → Journal → New entry. Give it a date (defaults to today), a one-line title, and the body. Tag it (e.g. "sleep", "side-effect") so you can filter later. Photos, PDFs, and audio attachments up to 25 MB each are welcome — a photo of a rash or a wound healing over time is exactly what providers find useful.

Private vs shared entries

Each entry is private by default — only you see it. Toggle Share with my care team to add it to your chart so your provider can read it before or during your next visit. You can change the toggle later; once shared, the entry shows in their chart immediately.

Search across the journal from the top of the page. Filter by date, by tag (you can tag entries — "headache", "sleep", "side-effect") or by attachment type. Useful when you are getting ready for a visit and want to remember every time something happened in the last month.
