Pick the screeners your clinic uses, assign them per patient or panel-wide, and triage the responses inbox without missing a flagged score.

The Outcomes dashboard is downstream of two things: which screeners you have enabled, and which patients you have assigned them to. This article walks the assignment side and the response inbox so the dashboard has data to draw from.

Enable screeners for your establishment

Establishment → Surveys. Toggle on the screeners you actually use (PHQ-9, GAD-7, AUDIT, WHODAS, custom). Each enabled screener shows its score range, the threshold for "elevated", and the default cadence. Custom screeners are built with the form builder — see "Build intake and consent forms".

In any patient chart → Surveys → Assign. Pick the screener and a cadence: one-time, weekly, monthly, or aligned to visits. The first request goes out immediately by SMS, email, and portal notification — whichever the patient has enabled. Recurring assignments fire automatically until you stop them.

Response inbox - severity chip routes elevated scores to the urgent queue

Sidebar → Surveys → Responses. New completed responses sort to the top with a colored severity chip (green / amber / red) based on the screener's thresholds. A red chip means the score crossed the clinical-action threshold (e.g. PHQ-9 ≥ 20, or PHQ-9 #9 > 0 for suicidality) — those route to your urgent queue and notify the assigned provider.

In-room completion

For patients who would not complete a screener at home, hand them a tablet in the waiting room logged into kiosk mode. The kiosk only shows screeners assigned today and submits straight to the chart — no portal account needed. Useful for older or less digitally engaged patients.

Pausing or stopping a series

In the patient chart → Surveys → tap the assignment → Pause or End. Pausing keeps the schedule but stops sends; ending stops the schedule entirely. Past responses stay in the chart and continue to feed the Outcomes dashboard until they fall outside the window.
