How to add an addendum, when to amend, and the audit trail behavior.

Once signed, a note cannot be edited. Use an addendum (added context) or amendment (correction) — both append to the original.

Original note plus addenda and amendments render together - every signature is timestamped, nothing is rewritten

Open the note → Add addendum. Write the new content; sign. The original note plus addendum render together; both timestamps are visible.

Amendment

For corrections (e.g. wrong dose typed). Open the note → Amend. Mark the corrected line; the original remains visible with strikethrough next to the amendment.
